Goochland Powhatan Community Services is seeking a Full-Time Developmental Services Support Coordinator
Under supervision of the Case Management Supervisor, this position is responsible for providing case management services to support children and adults with Developmental Disabilities (DD). These services utilize a team approach and are coordinated within the agency, community partners, and the family to meet the needs of the consumers in supporting independent living. Case management includes assessing needs, providing outreach, connecting the consumer with services, acting as an advocate, and providing supportive counseling. The Case Manager is responsible for preparing documentation with accuracy and completeness. The typical case load will be 20-30 cases.
This position is responsible for providing support coordination to support children and adults with Intellectual Disabilities (ID) and/or Developmental Disabilities (DD). Support Coordination includes conducting assessments, developing person-centered plans, and assisting eligible consumers in accessing the most appropriate services to meet their individual needs.
This is accomplished through linking, monitoring, documenting, coordinating resources, and advocating for effective and responsive service provisions. Bachelor's degree in human services and experience with ID/DD population is required.

Salary range is $55,000- $69,031 based on education and experience.
