Position: Social Services Coordinator


Location: Hutchins, TX




Rate of Pay: $45,000




HERE'S WHAT YOU'LL BE DOING:


The Social Services Coordinator (SSC) is responsible for assisting offenders with transitional programming. The SSC develops and coordinates re-entry programs to ensure continuity of care for the offender and facilitates services for offenders with special needs.


JOB REQUIREMENTS

• Conducts and/or coordinates client assessments to determine needs and service plans.
• Utilizes assessment data to determine the focus of an offender's pursuits in the RRC, i.e. employment, educational/vocational training, volunteer work, etc.
• Utilizes assessment data and input from resident and staff to determine if resident has any special needs for services, e.g., significant medical or mental issues, substance abuse treatment, specialized counseling, or other needs that could be served by community resources.
• Establishes linkages with agencies and service providers in the community that are available to meet offender needs and facilitate re-entry.
• Sets up and maintains a database of agencies and service providers most likely to be utilized for offenders' re-entry and ongoing needs.
• Educates offenders on service availability, application procedures, service locations, to facilitate appropriate linkages.
• Assists offenders in setting appointments and initiating contacts as needed.
• Coordinates the delivery of the Transitional Skills Program to all appropriate offenders, utilizing the Transitional Skills Journal from the Change Companies.
• Documents services regularly as prescribed by contract and Performance Work Statement, and maintains complete and current case files and records.
• Participates in weekly client staffing meetings.
• Assumes On-Call Supervisor responsibilities as required.
• Promotes an organizational culture that fosters passion for the mission, cooperation, open and frequent communication, teamwork, and is in alignment with the organizational vision.
• Attends all required meetings and trainings.
• Requires regular attendance and punctuality during the scheduled shift to perform the above essential functions.
• Performs other duties and special assignments as directed.

QUALIFICATIONS

• Bachelor's degree in social or behavioral sciences from an accredited college or university plus one year of experience working in the social services field in a relevant position.
• Valid Texas Drivers' License, clear MVR and ability to be insured.
