Your Role and Responsibilities

As a Home Health RN Clinical Liaison, you will play a vital role in facilitating seamless transitions for patients from acute to post-acute care. Your key responsibilities will include generating and managing referrals from hospitals, cultivating strong relationships with hospital discharge planning personnel, and assisting with marketing and sales initiatives. Your expertise, attention to detail, and dedication to patient care will be essential in ensuring our patients receive the high standard of care they expect while feeling comfortable and safe during their transition. Previous experience in a liaison role, home health, or case management is preferred.

Must-Haves for a Home Health RN Clinical Liaison:

Graduate of an accredited nursing program with an active RN license in IL
Minimum of 2 years of nursing experience
CPR and BLS certification
Knowledge of state and federal home health regulations
Clinical proficiency in nursing specialty, strong communication skills, and compassion
