Job Title: Licensed Social Worker

Position Overview: As a vital member of our team, the Social Worker will play a crucial role in planning, developing, and implementing our facility's social service programs. You will ensure that the emotional and social needs of our residents are met in accordance with federal, state, and local standards, as well as our internal policies. Your efforts will help create an environment where residents can achieve their highest level of well-being.

Key Responsibilities:

• 
  Program Development: Assist the Social Service Director in designing and executing social service programs tailored to the needs of residents.


• 
  Collaboration: Work closely with administration, medical, nursing, and other departments to strategize and enhance social services.


• 
  Policy Management: Help develop, implement, and review department policies and procedures, ensuring compliance with regulations and best practices.


• 
  Resident Support: Guide residents in achieving self-care, independence, and well-being through tailored social services.


• 
  Community Engagement: Participate in community planning to align facility services with resident and family needs.


• 
  Documentation: Maintain accurate progress notes and records, tracking residents' responses to treatment plans.


• 
  Resource Coordination: Connect residents with community resources and assist in scheduling necessary outpatient appointments.


• 
  Discharge Planning: Collaborate with agencies and individuals to facilitate seamless discharge processes for residents.


• 
  Training and Development: Contribute to training programs focused on the social and emotional needs of residents.


• 
  Budget Assistance: Aid in the preparation and management of the department's budget.


• 
  Compliance: Report any suspected fraud or unauthorized disclosures of resident information, ensuring confidentiality is maintained.

Qualifications:

Education:

• Bachelors degree in social sciences or related.


• Licensed Social Worker as such by the California Board of Behavioral Science Examiners.

Experience: Prior healthcare experience is a plus.

Skills:

• Strong written and verbal communication abilities.


• Proficient in interpreting and analyzing information.


• Ability to solve practical problems in diverse situations.


• Familiarity with healthcare regulations and social services.

Physical Requirements:

• Ability to lift and/or move up to 25 pounds occasionally.


• Frequent use of office equipment (computer, copier, etc.).


• May require assistance in emergency situations.

Work Environment:

• The work environment is generally low to moderate noise, conducive to focused work.
