Clinical Nurse Specialist - Post Acute Care Program Coordinator:




About this role: Collaborates with the VP of Post Acute Care in exploring and designing post-acute care strategies and solutions necessary to care for patients along the continuum. Manages the Post Acute Care Network (PAN) quality and safety program by collecting, compiling and displaying performance data for quarterly meetings with external partners, such as skilled nursing facilities, home health and long term acute care hospitals. Facilitates performance improvement initiatives across settings of care. Supports specialized and complex post-acute referrals by conducting patient specific clinical and behavioral assessments used for care transitions and identifying the most capable setting of care. Collaborates with the health system care teams and external post-acute partners in care protocols, communication hand-off tools, education and training that is used to connect and communicate care plans along the continuum

CNS Job Summary:




A Clinical Nurse Specialist (CNS) is a registered nurse who have graduate level nursing preparation at the master's or doctoral level as a CNS. They are clinical experts in evidence-based nursing practice within a specialty area, treating and managing the health concerns of patients and populations. The CNS specialty may be focused on individuals, populations, settings, type of care, type of problem or diagnostic systems subspecialty. CNS practices autonomously and integrate knowledge of disease and medical treatments into the assessment, diagnosis, and treatment of patients' illnesses. These nurses design, implement and evaluate both patient-specific and population-based programs. CNS provides leadership in advancing the practice of nursing to achieve quality and cost-effective patient outcomes as well as alternative solutions that address system problems and/or patient care issues. In many jurisdictions, CNS's, as direct care providers, perform comprehensive health assessments, develop differential diagnoses, and may have prescriptive authority, (but prescriptive authority is not mandatory). Prescriptive authority allows them to provide pharmacologic and non-pharmacologic treatments and order diagnostic and laboratory tests (associated with one's specialty) in addressing and managing specialty health problems of patients and populations. CNS serves as patient advocates, consultants and researchers in various settings. (ANA, 2004, p15). Specialty Practice: The context for Clinical Nurse Specialist practice may be defined by: population (pediatric, geriatric), type of problem (pain, wound management), setting (critical care unit, operating room), type of care (rehab, palliative), or disease/pathology/medical specialty (diabetes, oncology). Specialization in nursing practice includes in-depth graduate study of relevant theories and scientific knowledge, related to a select patient population or aggregate group, which promotes the ability to analyze and refine nursing practice and to generate clinical practice knowledge. Reporting Relationship The CNS position reports to the Director of the clinical unit(s) in a line relationship. CNS has an indirect relationship with the Vice President for Patient Care Services, Chief Nursing Officer. This dual reporting relationship is critical for the CNS to function optimally across the three spheres of practice: patient, nursing staff, and system. Communication among the CNS, CNO, and Director facilitates mutual expectations and priority setting allowing the CNS to enact his or her role at a unit and system level. Other duties as assigned.



EDUCATION DESCRIPTION:



Master's degree in Nursing is required.


SPECIAL SKILLS DESCRIPTION:



Expertise in a specialty; excellent verbal and written skills.


LICENSURE DESCRIPTION:



Licensure to practice nursing in Wisconsin; Certification as a Clinical Nurse Specialist required within 1 year of employment; Certification within a clinical specialty area preferred (Incumbents in the position prior to June 2011 may substitute the CNS certification requirement with a minimum of 5 years work experience as a CNS). American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required within 90 days.
