Job Description:

Job Title: Coach/Social Worker

Location: South Bay LA Region (Torrance, Long Beach, Hawthorne, Compton, South LA)

Work from home with field visits. Monthly in-office meetings in San Fernando, with an option to use Lynwood office for mail or other tasks.

Schedule: Monday to Friday, 8:30 AM - 5:00 PM

Overview:

Join a dedicated team as a Coach/Social Worker, supporting patients transitioning from hospital care back to their homes. This role combines client advocacy, case management, and home visits, aiming to improve patient outcomes by ensuring follow-up care, access to community resources, and addressing potential health risks. This is an excellent opportunity for someone passionate about helping others, with a strong sense of accountability and time management.

Responsibilities:
  1. Manage caseload of 12-20 clients weekly, conducting home visits and follow-up calls.
  2. Provide post-discharge support to ensure patients follow care plans and prevent health risks.
  3. Conduct in-home or telephonic assessments, creating care plans in collaboration with health care providers.
  4. Schedule and conduct home visits within 48 hours of discharge to assess living conditions and ensure patient safety.
  5. Link patients to community resources, such as home health services, physical therapy, or social services.
  6. Use motivational interviewing techniques to guide patients toward healthier lifestyles.
  7. Document home visits and assessments, sharing critical information with the care team.
  8. Ensure timely coordination of follow-up appointments with primary care physicians.
  9. Work closely with hospital and community-based referral networks to facilitate patient care.
  10. Maintain client files, ensuring all documentation is up to date and in compliance with regulations.


Qualifications:
  • Bachelor's Degree in Social Work, Case Management, or a related field preferred but not required.
  • Some field experience is desirable.
  • Strong time management, communication, and technology skills.
  • Valid driver's license, insurance, and ability to travel for home visits within the assigned territory.
  • Must be comfortable working independently and managing a flexible schedule.

Territory:

Primarily the South Bay area, including Torrance, Long Beach, Hawthorne, Compton, and surrounding regions.

  • Not pursuing clinical hours (LCSW or Master's) to ensure alignment with the long-term nature of the role.
