We are looking for a skilled, professional, Licensed Mental Health Practitioner to join our CFS Clinical Team. The successful candidate will impact the lives of delicate youth while supporting our therapeutic, restorative, and complete approach to their care. Work in this role is completed in a non-institutional setting.

In this role, the Behavioral Health Practitioner will be part of the Therapeutic Family Care Team:




  • Performs Licensed Mental Health Practitioner level professional work in determining youth's needs and providing complex therapeutic services.

  • Completes behavioral health assessment.

  • Leads a care coordination team for each eligible youth.

  • Serves as a member on interdisciplinary teams to participate in care plans for assigned youth, as applicable; provides clinical guidance to case managers, care team members, direct staff, practicum students, and/or volunteers.

  • Work is performed under the supervision of a CMSW Supervisor, Nurse Administrator, or Administrator. Performs related work as assigned.



For clinical programs with assigned patients:


  • Provides clinical care coordination consultation to social work and social work support staff.

  • Conducts comprehensive behavioral health assessments on an assigned caseload, as applicable.

  • Conducts individual, family, and provider team meetings to assist clients and families in care coordination , as applicable.

  • Compiles social and behavioral information from various sources. Serves as a member of an interdisciplinary team; provides information regarding a youth's social history and psychosocial assessments and addressing specific youth's needs; participates in the development of youth's active care coordination plan.

  • Monitors and evaluates youth's progress toward established goals; provides recommendations to members of an interdisciplinary treatment team to adjust a youth's care coordination plan.



For clinical programs with quality oversight:


  • Conduct research of medical records, case files, reports, journals, and other pertinent documentation to verify compliance with federal regulations.

  • Serves as liaison with service agencies consulting on member cases to ensure efficiency and continuity in providing services.

  • Conducts follow-up assessments and evaluations and provides assistance in resource development and in policy and program development and evaluation.




Requirements/ Qualifications:




Minimum Qualifications:



Behavioral Health Practitioner I:Provisional license to practice as a mental health practitioner in Nebraska.


Behavioral Health Practitioner II: Nebraska License as Mental Health Practitioner.


Behavioral Health Practitioner III:Nebraska license as Mental Health Practitioner or Nebraska license as Alcohol and Drug Counselor. AND Nebraska Provisional License as Mental Health Practitioner or Nebraska Provisional License as Alcohol and Drug Counselor.


Behavioral Health Practitioner IV:State of Nebraska licenses as Mental Health Practitioner and Alcohol and Drug Counselor.



Legal Requirements: Licensure requirements for all levels pursuant to Nebraska State Statutes 38-2122, 38-2124, and 38-316.



Hiring Rate: Pay commensurate with licensure. PLMHP: $24.1341, LMHP: $28.312, LMHP/PLADC: $30.435 LMHP/LACD or LIMHP: $32.717.



Position Location: Position available statewide, contingent upon available
