Job Description:

Summary

  • Provides a support to patients, family members and visitors in the Emergency Department.
  • Identifies patient concerns and facilitates resolution of concerns from point of Triage through disharge.
  • Coordinates the flow of the department while supporting families, visitors and patients.
  • Provides support to the families of patients and communicates the availability of community resources.
  • Maintains records, statistics and documentation.
  • Contributes to the discharge planning process through referrals to home care agencies/equipment companies, contact with insurance carriers and completion of appropriate forms.
  • Other duties: Helping patients obtain rides, set up cabs, bus passes, Contacting family members, Contact nursing to ensure patient is presentable and able to/willing to see family, Assisting homeless patients with securing shelters, Ensuring visitors are screened properly and are not on the do not visit list, Assist in keeping property room presentable and calling patients that have property left in the room, Bringing patients/family water, blankets (helping with basic needs).

Minimum Qualifications
  • Associate's degree or the equivalent combination of education and/or experience.
  • At least one year experience in a customer service or retail environment. Experience in a health care environment preferred.

Location and hours: This position reports on Site to Sinai Grace hospital 8:30 to 5pm every day. Assignment will be 6 - 12 weeks in length.
