Job Description:

East Bay Post-Acute is looking to expand its team! We are a 4-star facility located in Castro Valley, CA right next to BART, freeway and most importantly walking distance from Starbucks!

We are looking for Licensed Vocational Nurses (LVNs) :

• FULL TIME- AM SHIFT


• FULL TIME - PM SHIFT


• FULL TIME - NOC SHIFT


• Starting at $34 per hour

JOB FUNCTIONS:

• Make daily rounds on assigned units to observe, assess, or interview residents verifying information for or from ongoing shifts, monitor regulatory compliance, and determine staff assignments or completion of assignments


• Document Med Pass in the electronic MAR system (PCC), daily charting, or COC for residents.


• Administer medications, and treatments and provide direct care to residents on the unit according to physician orders and in compliance with facility policies and procedures


• Perform physical observation of residents as indicated by change in condition or as required by regulation. Complete assigned sections of resident observation instruments used in the facility.


• Coordinate care and delivery of services with all disciplines, outside agencies, and community providers as required


• Respect the rights of residents regarding freedom of choice, consent for care and services, refusals of treatment, and implementation of advance directives if any.

REQUIREMENTS:

• Active LVN license
