Position Summary: We are seeking a compassionate and proactive Nurse Supervisor to lead our nursing team on weekends in delivering high-quality care to our residents. This role involves overseeing daily operations, ensuring effective communication, and fostering a safe and supportive atmosphere for both residents and staff.
Key Responsibilities:
Resident Engagement: Communicate regularly with residents and their families about Plans of Care, physical therapy (PT), occupational therapy (OT), and other treatment protocols to ensure understanding and comfort.
Team Collaboration: Participate in daily morning meetings to enhance communication and coordination among the healthcare team.
Care Management: Address residents' questions regarding their care, treatment plans, and illness progression to promote a sense of security.
Monitoring and Adjustment: Assess and adjust residents' care based on evolving needs and conditions, ensuring optimal outcomes.
Work Organization: Prioritize daily tasks by evaluating the needs of new, current, and discharging residents within your areas of responsibility.
Documentation: Complete necessary documentation and perform utilization review activities to provide timely and cost-effective care.
Care Coordination: Facilitate care coordination with residents, care providers, financial services, and third-party payers.
Admissions and Discharges: Oversee all admission and discharge processes, ensuring a smooth transition for residents.
Referral Management: Coordinate referrals to and from our facility.
Professional Interaction: Build positive, respectful relationships with residents, families, staff, and professional colleagues.
Qualifications:
Education: Bachelor's Degree in Nursing or Social Work. Registered Nurse (RN) license preferred; Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) also acceptable.
Experience: Minimum of two (2) years of clinical nursing experience preferred. Knowledge of Medicare, Medi-Cal, and Medicaid programs is a plus.

Excellent communication and interpersonal skills to foster collaboration.
Strong organizational and time management abilities to navigate a high-volume, fast-paced environment.
Proficiency in using computers and maintaining accurate documentation.
Certificates, Licenses, and Registrations:
Valid LVN/LPN or RN license in the state of practice.
Certification as a Certified Case Manager (CCM) is a plus.
Commitment to ongoing education and maintaining good standing with the Department of Public Health.
Physical Demands:
Ability to stand and walk frequently; occasional sitting.
Frequent reaching, pushing, and pulling; lifting up to 50 pounds.
Must have close, distance, and peripheral vision abilities, as well as depth perception.
Work Environment:
Generally low to moderate noise levels, with reasonable accommodations available for individuals with disabilities.
