Our organization is seeking qualifiedHospice LPN Case Manager (LPN)to work with our geriatric population in their own homes. Responsibilities include comprehensive assessments,individualized care planning, patient and family education, interdisciplinary care management, and palliative care all within the home setting. The nurse will work as a liaison between physicians, families and the administrative nursing team to ensure a continuum of care.
*The ideal candidate will have Hospice experience and the following qualifications:
Willing to travel to multiple locations throughout the Grand Rapids areaValid RN License in good standing with the State of MichiganCPR/BLS CertificationAbility to work independently as well as within a team environmentExceptional interpersonal & communication skillsOutstanding organizational and self-management skillsProficient computer skillsAuto Insurance and reliable transportation1 year experience in hospice setting
