LIFE is a program that serves county residents living in Armstrong, Butler, Beaver and Lawrence Counties. It is an all-inclusive comprehensive program that helps individuals maintain their independence in their home and community through supportive services. The LIFE programs offer an array of person-centered services from medical care to adult day supports with the primary goal of allowing individuals to remain in their own home.
Under the supervision of the Director Clinical/Homecare Services, within an Interdisciplinary Care Team setting, assumes responsibility of participant care in the Clinic or in the participant's home by performing professional nursing duties and assuring the completion of Program Assistant duties. Cooperates with all disciplines to assure high quality participant care. Responsible for the implementation of nursing care plan and coordination of nursing care with others. Oversees duties of Program Assistants as needed.
Qualifications
Graduated from an accredited School of Nursing and must possess a current Practical Nurse license (LPN); in good standing, from the Commonwealth of Pennsylvania.
Must have (1) year experience with the frail or elderly.
Must have dependable transportation with a valid driver's license.
