Under the direction of the Emergency Mobile Respite Clinical Specialist, the Emergency Mobile Respite Case Worker will provide "wrap around services" to clients who require intensive short term care coordination to allow them to maintain community tenure. Responsible for facilitation and participating in individual treatment planning including supporting the client in gaining resources and supports in order to successfully maintain themselves in the community.
Provides active efforts to engage clients referred through Emergency Services Program:
Meets with clients in the Emergency Department in order to provide support for diversionary activities
Provides direct contact with individual on regular basis commensurate with the individual's needs
Works with the individual's team to create and implement a treatment plan that addresses the individual's needs
Supports clients to address needs and access resources including; housing, transportation, money management, utilizing of community resources, medical resources, outpatient support, entitlements, employment, and other natural supports
Refers individuals to longer term supports as needed
Transports individuals as appropriate
Provides intensive short-term support to help stabilize behavior
Utilizes EBP in order to engage clients that are pre-contemplative
Provides individual and family education
Facilitates communication between PCC, medical providers, and behavioral health providers
Participates in case conferences, family consultations, and liaison functions
Required Skills
BA degree in psychology or related field preferred
CPR/First Aid certification
Computer proficiency
Valid driver's license and access to personal vehicle to provide transportation of consumers
Required Experience
Previous experience working with Behavioral Health populations strongly preferred
Experience in community social services strongly preferred
