Sonoma Post Acute in Sonoma, CA, is seeking a passionate and motivated Assistant Director of Rehabilitation to join our team. We are looking for either a Physical Therapist Assistant (PTA) or a Certified Occupational Therapy Assistant (COTA) to help lead our rehabilitation department. Leadership Role: Help manage and guide the rehabilitation team in delivering exceptional care.

Key Responsibilities:
Assist the Director of Rehabilitation in overseeing day-to-day operations of the rehabilitation department.
Lead therapy sessions and work directly with residents to achieve their rehabilitation goals.
Ensure compliance with all regulatory requirements and maintain documentation standards.
Collaborate with the interdisciplinary team to provide coordinated care.
Mentor and support fellow therapists and therapy assistants to ensure high-quality care.
Qualifications:
Licensed Physical Therapist Assistant (PTA) or Certified Occupational Therapy Assistant (COTA) in the state of California.
Previous experience in a skilled nursing or rehabilitation setting is preferred.
Strong leadership, communication, and organizational skills.
A passion for patient care and improving rehabilitation outcomes.
