Job Description:

Location: REMOTE

Salary: $38.00 USD Hourly - $40.00 USD Hourly

Description: Our client is currently seeking a Case Manager RN licensed in Nevada for a remote position.
The job is work with 4 different facilities near Las Vegas.



This job will have the following responsibilities:

• Assess and create discharge plans to ensure patient receives quality medical care. Implements care strategies that are individualized for patients and directed toward the most appropriate and the least restrictive level of care.
  
  


• Collaborate with patient, family, and health care providers to develop an individualized plan of care that encompasses both acute care episode and post hospital discharge plan.
  
  


• Advocate for patients and families as needed to ensure the patient's needs and choices are represented and supported by the health care team.
  
  

Qualifications & Requirements:

• Minimum two years of varied clinical experience in a hospital setting with an emphasis on case management or utilization management. Experience in a managed care organization
  
  


• 
  Must possess an active Nevada RN license
  
  
  

The client is looking to hire on a temp to perm basis.
