Job Overview

The Manager, Nursing Support Services (PACE) shall have the responsibility and authority to carry out the policies, procedures, and standards of the PACE Center(s) regarding the assessment, planning, organizing, and directing home care services for PACE Program participants and ensures adherence to and compliance with licensing and regulatory requirements. Responsibilities include training and oversight of planning, development, service, implementation, and coordination of related services and the overall leadership and administrative operation of home care services focusing on providing quality cost-effective care in the home setting. This individual shall be the liaison between AltaMed PACE and the Home Caregiver Services Vendor to ensure contract adherence, quality control and process improvements for the delivery of home care services as needed by participants. Shall be responsible for budget and fiscal effectiveness of the home care services through collaboration with PACE Center leaders, Nursing and Operations. This individual builds from the resources of the PACE program to plan and direct services to meet the needs of the individual and their family within the home setting. The Manager, Nursing Support Services (PACE) respects and promotes the mission, vision, and values of AltaMed PACE.

Minimum Requirements

  • Graduation from an accredited Registered Nursing school, Bachelor’s degree (required).
  • Advanced degree in nursing, preferred.
  • California Board of Registered Nursing required.
  • Management, supervision and/or leadership experience in a Healthcare environment required.
  • Minimum of 5 years’ experience in working with the following: healthcare organization(s) with either Title XXII (licensing regulations) and/or the elderly population; and experience working in teams with, but not limited to the following disciplines: nursing, social work, psychology, recreational therapy, occupational therapy, physical therapy, speech therapy, dietetics, medicine and/or gerontology.
  • Minimum 1 year experience working with frail elderly (Geriatric) population as required by CMS PACE regulations. Experience and knowledge regarding the physical, mental and social needs of geriatric patients highly desirable. Experience with physically or mentally impaired adults and/or geriatric population.
  • Current CPR/BLS certification required.
  • Knowledge of PACE, state, federal, local and accreditation regulations for the delivery of home services.
  • Experienced nurse with an emphasis on public health or home health.

Compensation

$118,898.86 - $148,623.58 annually

The range is subject to change.
