Position Summary

The Primary Care Pharmacist is responsible for the interpretation of medical orders and counseling patients on proper medication administration and storage. The Primary Care Pharmacist must have strong communication with fellow colleagues to ensure the expectations of Patient Aligned Care Team metrics are met.


Core Responsibilities

  • Evaluates and enters provider's orders/prescriptions for appropriateness of therapy.
  • Evaluates incompatibilities, side effects and interactions of drugs and drug products, and resolves drug-related questions with the prescriber.
  • Documents all medical information into the patient's electronic medical record utilizing the VA's VISTA/CPRS electronic patient record system.
  • Maintains confidentiality of veterans' data and information.
  • Provides patient consultation by reviewing patient profiles and medication orders.
  • Provides drug information consult to providers, staff, and other healthcare professionals.
  • Maintain a record of all C2 prescriptions ordered by providers in your clinic.
  • Maintain a record of all 10-day urgent prescriptions prescribed by our clinic provider and obtained by the patient from a local pharmacy vendor.
  • Evaluates patients' records from medical providers outside the VA and works with these patients utilizing rules set forth by the VA for co-managed care.
  • Completes any and all clinical reminders due at the time of each patient visit.
  • Completes the documentation of the medical record within twenty-four hours of patient encounter.
  • Complies with all VA guidelines in regard to appropriate and timely clinical documentation and response to patient requests.
  • Complies with all VA and company training requirements.
  • Fulfill compliance requirements of the Office of Inspector General (OIG), Joint Commission(JC), Environment of Care (EOC) oversight, lab compliance and other related items.
  • Remain focused on achieving excellent clinical outcomes through the specified VA guidelines.
  • Must provide excellent customer service to each veteran, both in person and over the phone, as well as to fellow colleagues and clinic visitors.
  • Participate in the clinic's outreach events to help support the clinic's enrollment initiatives.
  • Support patient enrollment and retention by providing guidance, when necessary, regarding scheduling, follow-up visits or nurse visits.
  • Embrace and support new initiatives, whether clinical or operational.


Requirements:

Qualifications

  • Graduate of an accredited school of pharmacy with a R.Ph or Pharm.D degree
  • Certification by applicable professional organization
  • Minimum one-year clinical experience (three years preferred) as a pharmacist within the last 4 years in a related primary care or ambulatory care setting; government healthcare environment preferred. Specific requirements could vary based on individual VA contract.
  • Must be credentialed and remain in good standing through the Veterans Health Administration
  • Must comply with and maintain all requirements for a valid, unrestricted license in the state of desired employment, or in any U.S. state or territory, depending on VA contractual requirements
  • Current certification in Basic Life Support (must be renewed periodically as specified by the certifying agency – AHA valid for two years, e.g.) and in ACLS as specified by individual VAMC contract
  • Current, unrestricted Drug Enforcement (DEA) registration
  • Proficiency in written and spoken English
  • Strong computer skills
  • Energetic and optimistic demeanor
  • Strong service mentality and a focus on achieving all aspects of defined service standards
  • This is considered a safety sensitive position.
