The Population Health Specialist is responsible for assessing, implementing, and monitoring healthcare options and services with the goal of engaging patients in accessing health services, thereby increasing the likelihood of improvement of the health status of identified populations.
This is a full-time, benefit eligible position working Monday-Friday, 8-hour shifts. This position works remotely but candidate may be required to be on-site one at times. *Position open to Missouri residents only*
Responsibilities Include:
Patients not currently in the practice:
Provide outreach to patients not currently receiving Health Center services using contact information provided by health plans for outreach activities
Encourage identified patients to access Health Center lines of service
Patients currently in the practice:
Use reports to proactively prevent care gaps, address existing care gaps, and provide follow-up by scheduling patients to address any identified care gaps
These could include specific health needs i.e. diabetics, pediatric patients, etc.
Follow-up with patients that miss preventative appointments and reschedule
Utilize available information to evaluate the effectiveness of services provided in producing the desired outcomes
Assist in a variety of initiatives and programmatic performance monitoring as determined by supervisor and senior management
Overall essential functions:

Educate patients to the services offered by the organization that can improve their health
Prioritize multiple tasks and demands while demonstrating an ability to foster relationships with patients and their caregivers to increase the likelihood of patient retention
Tracks, manages, documents, identifies, and reports patient data to contribute to program performance and fiscal outcomes, and improved patient outcomes
Demonstrates an impact on an increased number of kept and scheduled patient appointments
Demonstrates improvement in clinical quality measures through an increased number of wellness visits, screenings, chronic disease management follow-up, etc. to support overall clinical quality measure performance and value-based payment initiatives
Comply with all federal and state privacy laws
Proficient in Microsoft Office products, electronic health record, and electronic mail
Performs other duties as assigned by the Director of Population Health or senior management
High School/GED required
Bachelor's degree preferred
WORK EXPERIENCE/TRAINING/ADDITIONAL REQUIREMENTS
At least three (3) years' experience required
