Hours/Location
As a hybrid opportunity, the position entails onsite coverage 4 days per week (days can be flexible depending on the candidate's schedule) and remote coverage 1 day per week. Our social workers are expected to rotate working on-site at either of the following locations:

Due to the hybrid nature of the role, we ensure that our employees receive the required technology and training to be proficient and independently productive in all job responsibilities regardless of work location. Employees are responsible for designating a workspace within the remote work location that is private, safe, ergonomic, and free from distractions for all hours worked.
The Opportunity
Our Clinical Social workers are integral members of the ambulatory primary care team, working closely with the clinical and administrative staff to enhance the delivery of patient care services along the continuum of care. This is a collaborative approach to providing the best care! This employee will identify high-risk psychosocial factors of patients, help primary care staff understand the influence of those factors upon the course of medical care, and assume care coordination responsibility of the patients. Depending on each case, there are times when the social worker will provide treatment or intervention. In other cases, the social worker will be working with the treating outpatient clinicians (in or outside of our system) helping to ensure the treatment is focused, effective, and carried through.

Qualifications
Qualifications:
When hiring, we look for candidates who possess not only the relevant skills and competencies, but also positive attitudes, emotional intelligence, and genuine passion for this work.
Current LICSW or LCSW in MA required
Minimum of Masters-level degree program in social work required
Minimum of 2 years clinical experience. Other experience may include post-acute care settings or care coordination/case management experience.
