Key Responsibilities:
- Coordinate patient care plans in collaboration with the interdisciplinary team
- Act as a liaison between healthcare providers, families, and patients
- Monitor patient progress and adjust care plans as needed
- Ensure compliance with regulatory standards and company policies
Qualifications:
- Valid RN or LPN license
- Strong communication and organizational skills
- Experience in case management or similar role in a skilled nursing or rehabilitation setting preferred
