Job description
Job Summary:
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. As a Case Manager, you will be responsible for coordinating and providing comprehensive patient care, ensuring the highest quality of care and patient satisfaction.
Duties:
- Conduct patient assessments and medical history review
- Collaborate with physicians and healthcare professionals to develop individualized care plans
- Monitor and evaluate resident progress, adjusting care plans as needed
- Provide education and support to residents and their families regarding their healthcare needs
- Coordinate admission and discharge planning and ensure smooth transitions of care
- Maintain accurate and up-to-date medical records, adhering to HIPAA regulations
- Collaborate with interdisciplinary teams to ensure effective communication and coordination of care
- Utilize case management principles to optimize patient outcomes
-initial and periodic evaluation, at least once every 12 months, of the needs of a resident and of the capability of the facility program to meet those needs;
-orientating a new resident and family to the daily routine;
- assisting each resident to adjust to life in the facility;
- Identify persons in need of services and assist external service providers in establishing a relationship with these residents;
-Work with these service providers in executing a plan for service for individual residents; and
-Assist in arranging for transportation as necessary to ensure that residents are able to attend required services provided in an external location.
-Each resident shall be provided such case management services as are necessary to support the resident in maintaining independence of function and personal choice.
-The operator shall establish a system of recordkeeping which documents the case management needs of each resident and records case management activities undertaken to meet those needs.
Experience:
- Minimum of 1 year of experience as a Registered Nurse
-Strong knowledge of resident t assessment techniques and clinical skills
- Familiarity with admission and discharge planning processes and resources
- Proficient in electronic medical record systems
Skills:
- Excellent communication and interpersonal skills
- Strong critical thinking and problem-solving abilities
- Ability to work collaboratively as part of a multidisciplinary team
- Detail-oriented with excellent organizational skills
- Ability to prioritize tasks and manage time effectively
