The Advanced Practice Clinician will provide comprehensive medical care to patients served at Choptank Community Health System (CCHS), in accordance with the policies and procedures and philosophy of CCHS. All duties and responsibilities are performed across life cycle (infants, children, adolescents, adults, and geriatric), as applicable.
Duties/Responsibilities:
Assess and manage patients including conducting history, performing physical examinations, and developing treatment plans for acute and chronic health problems and address preventive health needs appropriate to that clinician's training, experience, and specialty, taking into consideration the ambulatory nature of the health center. Order and review lab and x-ray tests in accordance with evidence based and best practice recommendations
Support services provided in Health Centers and the community/school-based health center programs
At all times interact with patients and staff in accordance with CCHS Standards of Behavior and Code of Ethics
Complete patients encounter medical record and complete tasks in a timely manner to include; dropping charges on day of service, completing tasks within 48 hours, addressing requests for prescription renewal within 72 hours and completing and signing all patient encounters within 72 hours of the date of service.
Make appropriate referrals to specialists, subspecialties, hospital emergency rooms, hospital clinics, etc., when good medical judgment dictates such referrals.
Provide first-aid to patients but may refer serious injuries or severely acute conditions requiring hospital care to local hospital emergency rooms or in-patient facilities, as appropriate, after stabilizing the patient as clinically necessary at the health center.
Participate in medical audits, performance improvement, and peer review activities, and participate in appropriate CCHS committees, when necessary.
Provide in-service training sessions for other CCHS staff, as necessary.
Attend CCHS staff meetings unless direct patient duties prevent such attendance.
Serve as a resource and consultant to other advanced practice clinicians, and review problem cases with the advanced practice clinicians as needed. When assigned, audit charts and help assure that clinicians are providing care within generally accepted current standards.
Take night call and weekend call in accordance with CCHS policies and procedures.
Must obtain and maintain appropriate licensing, credentialing, and privileges according to CCHS policies and procedures. Complete required CME yearly relevant to Board Certification and practice- supported with an annual CME stipend.
Ensure proper and current charting in the medical records.
Participate in educational programs and classes for patients and staff.
Participate in Performance Improvement, Risk Management, and Peer Review activities at CCHS. This includes peer review audits, chart reviews, and employee performance evaluations.
Coordinated Care/Patient Centered Medical Care Management: Works with all members of the Health Care Team towards achieving coordinated patient centered care. Demonstrates knowledge of the goals and objectives of CCHS's patient care management plan. Effectively able to perform and communicate individual task within role in the process.
Participate in CCHS medical activities at any or all sites operated by CCHS including high school satellite clinics, etc., appropriate for specialty, in accordance with CCHS policies and procedures and needs.
Participate in CCHS outreach activities in accordance with CCHS policies and procedures.
Maintain client confidentiality at all levels of interaction in strict compliance with State and federal laws and regulations.
Perform other job-related duties deemed necessary by the Supervising Director Chief Clinical Officer.
Regular, reliable attendance is a requirement of this job.
Restrictions:
May not have any private practice or provide medical services outside of CCHS without the written approval of the CCHS CEO. Any other medical practice may not interfere with duties and responsibilities to CCHS and its patients.

Education and Experience:
Graduate of Accredited School
Board Certified
Primary Care specialist (Family, Pediatrics, Adult, Women's Health, or Midwifery).
Must have current NP or PA license to practice in Maryland.
Maintain ongoing professional education as mandated by state regulation.
Maryland CDS and DEA license and Basic Life Support.
