Job Description:
Join our Louisville East Post Acute Team! We are a 175 bed facility with 5 Units including Sub Acute Rehab, Pathways TBI, LTC and Memory Care!
Treat Residents as directed by the Occupational Therapist. Record daily treatment notes and weekly progress notes per OT Board.
Communicate with supervisor and other health team members regarding Resident progress and treatment goals.
