Salary: $75,000.00 USD Annually - $85,000.00 USD Annually

Description: Our client is currently seeking a Director of Discharge Planning

The Director of Discharge Planner is responsible for coordinating and facilitating the discharge process for patients within the facility. The Discharge Planner ensures a smooth transition from the nursing home to the patient's next level of care, whether that is home, an assisted living facility, or another healthcare setting. This role involves comprehensive discharge planning, community service integration, and referrals to appropriate post-discharge services to support optimal patient outcomes.


This job will have the following responsibilities:

• • Conduct introductory discharge planning care conferences with patients, families, and the healthcare team to discuss and outline the discharge process.
  
  
  • Develop and implement individualized discharge plans for patients in the short-term rehabilitation unit. • Identifies patients for teaching, discharge, and extended care facility needs.
  
  
  • Join daily rounds with other team members to coordinate care
  
  
  • Coordinate and collaborate with physicians, nurses, therapists, social workers, and other healthcare professionals to ensure all necessary preparations are in place for a safe discharge.
  
  
  • Participates in team meetings that foster interdepartmental collaboration with the patient and their family as deemed necessary, this includes multidisciplinary meetings and Utilization Review/Case Management meetings. Provides input in such meetings regarding utilization management and discharge planning.
  
  
  • Applies utilization review criteria to assess and document the appropriateness of admission, continued stay, level of care, and readiness for discharge.
  
  
  • Maintains working knowledge of Medicare, Medicaid and private insurance company coverage for referred products and services.
  
  
  • Participate in discharge planning meetings with the patient and/or family to review patient progress and adjust discharge plans as necessary.
  
  
  • Communicate daily with admissions and interdisciplinary team regarding discharges from the facilities and manage the discharge dashboard/calendar ensuring is it up to date daily.
  
  
  • Ensures that a quality of care is maintained or surpassed by collecting quality indicators and variance data and reporting the data to the appropriate department; reports and identifies data that indicates potential areas for improvement of care and services provided within the system.
  
  

Qualifications & Requirements:

• • Knowledge of clinical services and insurance coverage preferred • Knowledge of discharge planning processes and community resources.
  
  
  • Strong organizational and interpersonal skills
  
  
  • Ability to determine appropriate course of action in more complex situations
  
  
  • Proficient in electronic medical records (EMR) systems and discharge planning software
  
  
  • Ability to work independently, exercise creativity, be attentive to detail, and maintain a positive attitude
  
  
  • Ability to manage multiple and simultaneous responsibilities and to prioritize scheduling of work
  
  
  • Ability to maintain confidentiality of all medical, financial, and legal information
  
  
  • Ability to complete work assignments accurately and in a timely manner
  
  
  • Ability to communicate effectively, both orally and in writing
  
  
  • Ability to handle difficult situations involving patients, physicians, or others in a professional manner
