Description:The Advanced Practice Provider is responsible for providing comprehensive medical care in partnership with physicians. This role involves conducting physical examinations, diagnosing and treating illnesses, ordering and interpreting laboratory tests, prescribing medications, and providing patient education and counseling. The Advanced Practice Provider will also provide direct patient care, collaborate with other healthcare professionals, participate in quality improvement initiatives, and support administrative duties.
Ensures that care delivered to patients, meets/exceeds standards of care and is provided according to the mission, strategic plans and all internal/external regulations, policies, and procedures.
Functions independently and collaboratively with the physician to provide continuity of health care services for individuals, families, or groups.
Evaluates the health status, acute episodes of illness, and health care needs of individuals through emphasis on comprehensive history, physical exam, and psychosocial assessment.
Analyzes data to identify health/illness problems, the human responses to actual or potential health/illness problems, identifies variations from the norm in diagnoses emergent problems and complications.
Designs and evaluates individualized treatment plan. Consults with other members of the health care team on emergent problems for rapid disposition/resolution.
Coordinates care for optimal continuity of care across the health care continuum.
Facilitates patient/family participation in health care by identifying learning needs and providing specialized education to enhance decision making.
Exhibits professionalism by performing research, education, and leadership in clinical practice.
