Our Palliative Care Nurse Practitioner Will:
Practice independently within Nebraska's full practice authority scope, conducting comprehensive patient assessments, developing individualized care plans, and providing expert symptom management and pain control to improve quality of life for both patients and their families.Collaborate with interdisciplinary team members including nurses, social workers, and chaplains, as part of a dedicated care team.Maintain their own patient panel and manage care decisions autonomously.Thoroughly document patient care activities in electronic health records.Provide education and support to patients, families, and healthcare team members.Participate in an on-call rotation as required.
