Salary: $7,239.09 - $9,370.90 Monthly

SUMMARY

HFH is a division under the Department of Health Services that provides housing and services to unhoused residents of Los Angeles County. HFH serves individuals with complex medical and behavioral health comorbidities, and many have difficulties in performing activities of daily living (ADLs) and independent ADLs due to physical, cognitive, psychiatric, or addiction issues. The HFH Enriched Residential Care (ERC) program places homeless individuals with functional deficits into licensed adult residential facilities (ARFs) and residential care facilities for the elderly (RCFEs) and then provides technical assistance/quality assurance services to facility administrators to ensure that HFH clients receive high quality care and attention so that their health, quality of life, and community integration are optimized. 

Under the direction of the HFH Clinical Social Work Chief in the Department of Health Service (DHS), the HFH Clinical Social Work Supervisor will be a key member of the HFH clinical team and will provide clinical and administrative oversight of case management services that are provided to clients in the ERC program and will also provide some back up support the Mobile Clinic BH team who provides services to the unsheltered population. The Mobile Clinic is a street medicine multidisciplinary team that will provide medical and behavioral health care to people experiencing homelessness in the unsheltered setting. Direct patient care will be provided in a mobile clinic, encampments and on the streets.

The Clinical Social worker supervisor will oversee the BH staff and in partnership with the SW chief, plan, direct, assign, and evaluate the work of BH staff in provision of high-quality case management services. The Clinical Supervisor will confer with program managers, nursing, and ERC/ Mobile clinic leadership to plan new programs, workflows, and to effectively coordinate service delivery to clients, including the identification of training needs for staff. 

 

ESSENTIAL FUNCTIONS


  • Provides supervision and oversight to case management and behavioral health team members.

  • Clinical and administrative oversight of BH staffing; Supervision to include job assignment, performance management/evaluation and training of staff.

  • Provides oversight to ensure there is optimal care coordination for clients. This includes:

    • Coordination, facilitation & participation of care coordination meetings to ensure staff are supported in delivering high quality case management services.

    • Develops and implements methods for measuring effectiveness of services, collaborating with program managers in utilizing outcome evaluations to improve treatment service.

    • Work with staff to promote care integration with medical, nursing, program managers, and other partners and HFH team members to provide effective care coordination and case management services.

    • Review of care plans and goal setting for staff to ensure care delivery is appropriate, high quality, timely, and patient centered

    • Assists with the development of workflows and program policy and procedures.

  • Provides support to staff with debriefings, incident reports, quality review of work.

  • Plans and conducts staff orientation, continuing staff development and orientation of other disciplines.

  • Coordinates the activities of social work and case management services through interdisciplinary staff meetings, resolution of interdisciplinary procedural and personnel problems, and implementation of new services or programs.

  • Ensures appropriate training and competent service delivery related to case management services, behavioral health support and services, and addressing Social Determinants of Health (food insecurity, housing, etc.)

  • Work with staff to promote care coordination, care integration and collaboration with outreach teams and other treatment teams

  • Provides consultation to other multidisciplinary team members to support staff in delivery of care for those with chronic medical and behavioral conditions and to prevent acute care utilization through tertiary care.

  • In addition to the above responsibilities, the HFH Clinical Social Work Supervisor will also engage in direct patient care and conduct clinical social work activities for clients, as needed and for coverage purposes. This can include conducting patient assessments and providing counseling to patients when needed.

ADDITIONAL ADMINISTRATIVE FUNCTIONS


  • Assists with the identification of training needs and the development of staff.

  • Provide input regarding program, policies and procedures through individual case conferences, staff meetings and budget requests.

  • Participates in performance/quality improvement (PI/QI) activities and contributes to policy development as needed.

  • Shares knowledge and effective practices with other care team members. 

JOB QUALIFICATIONS

A Master's Degree in Social Work from a graduate school accredited by the Council on Social Work Education followed by four years of professional social work experience, two of which must have been in a medical setting functioning in an independent capacity providing highly specialized social work services. AND A valid and active license as a Licensed Clinical Social Worker issued by the California Board of Behavioral Sciences and a valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.

 

DESIRABLE QUALIFICATIONS


  • Clinical social work in homeless health care experience.

  • Experience/expertise in assessing and managing clients with dysfunctional SUD.

  • Experience/expertise in assessing and managing clients with severe persistent mental illness.

  • Experience/expertise in trauma-informed care and harm reduction principles.

  • Demonstrated teaching and communication skills with clinical and non-clinical staff.

  • Experience providing clinical and administrative supervision to other behavioral health staff.

  • Familiarity with DMH, DPH and community mental health and substance use programs and resources.

Certificates/Licenses/Clearances


  • A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.

  • Successful clearing through the Live Scan and the Health Clearance process with the County of Los Angeles.
