Schedule: Monday-Friday 8:00 AM-5:00 PM




Pay: $141,232-$150,862.40 Annually



The Clinical Home Care Director The primary purpose of the position is to plan, budget, organize, develop and direct the overall operation of the home health clinical services in accordance with current Federal, State and local standards, guidelines and regulations that govern the facility to ensure that the highest degree of quality care is maintained at all times in accordance with the Budget and Operation Plans of the organization. Responsible for ensuring that an adequate level of services is provided to each client, documented appropriately and regularly evaluated.

  • Graduate of an accredited school of nursing and currently registered with the State Board of Examiners for Registered Nurses in the state where the facility is located.

  • Minimum of three (3) years administrative and supervisory experience in a long-term health care setting with knowledge of rehabilitative nursing.

  • Verbal and writing abilities necessary to communicate and work effectively with various levels of staff, clients, family members and the public, and representatives of the various regulatory agencies, and for required written documentation or reports.

  • Numerical ability necessary to make various computations in planning departmental objectives, keeping work records and schedules, preparing budgets, requisitioning supplies, and monitoring inventory control.

  • Ability to apply principles of personnel administration to interviewing, selection and supervision of employees, and in monitoring department activities.

  • Able to evaluate reports, programs, systems, and other data against both judgmental and verifiable criteria.

  • Able and willing to work flexible hours, such as during a staff shortage or emergency situation.




Preferred:



  • Certified Professional in Healthcare Quality (CPHQ) preferred.




Essentials:




  • Develops, and guides the ongoing implementation of the annual Quality Improvement and Compliance Plan

  • Ensures staff integration into the Quality Improvement process through Continuous Quality Improvement education and developing opportunities for input.

  • Analyses risk management data, and Quality Improvement activities data with the objective of identifying and, if applicable, correcting instances of Fraud, Waste, and Abuse.

  • Oversees the process to evaluate, and resolve medical and non-medical grievances and appeals by participants, and or their representatives, ensures information is reviewed for incorporating issues in development of Quality Improvement Initiatives and annual Quality Improvement and Compliance plan.

  • Facilitates the quality assurance/performance improvement process for various service areas within Senior Community Care

  • In conjunction with the Safety Committee has overall responsibility for Senior CommUnity Care's Emergency/Disaster planning, training, and implementation.

  • Conducts annual satisfaction surveys of the participants and caregivers of Senior CommUnity Care and reports findings.
