The Patient Navigator (PN) will serve as the bridge to care for Hepatitis C (HCV), HIV, and sexually transmitted infection (STI) prevention and care. This function includes linking and assisting patients in the receipt of insurance, patient payment assistance plans, and scheduling and attending appointments. The PN will also assist in the entry of patient encounters into the electronic health record as well as the States third-party billing system TN-WITS. The PN will receive training in pre-exposure prophylaxis (PrEP) navigation as well as HIV/HCV counseling and testing and will provide these services to patients in the clinic and through outreach. This position is 100% in-person on our Syringe Services Mobile Unit.
Daily Operations
Provides patient education, linkage to care and referrals regarding HIV/HCV and sexually transmitted infections as well as providing HIV/HCV testing.
Assist patients to receive indigent insurance as well as access to patient assistance plans for HCV treatment.
Manage client linkage to services across the three partner clinics.
Compiles and submits program reports as required.
Conducts patient navigator duties on the mobile SSP unit as needed.
Enters data into Health Record and third-party payor systems.
Conducts check-in meetings with client support members as necessary.
Provides Peer Recovery Support to continually engage the patients.
Maintains an ongoing log of communication with patients to facilitate treatment.
Participate in weekly project team meetings and staffing.
Performs other duties as assigned.

Required Education and Experience
A Bachelor's degree in Social Work or related field; or equivalent experience and training.
Excellent communication skills, with the ability to communicate effectively throughout all levels of the organization and community.
PC proficiency with working knowledge of Microsoft- Word, Excel, PowerPoint.
Excellent organizational and interpersonal skills.
Knowledge and ability to access community resources.
