Weekender with benefits; 3, 12 hour night shifts on the every other weekender program (paid at time and a half) plus one additional shift per pay period (works 48 hrs per pay period); holiday rotation required; must adhere to weekender plan guidelines.

JOB SUMMARY

The Patient Care Coordinator serves as the inter-department Nurse Coordinator for Acute Care, Women’s Health, OR and ER during evenings, nights and weekends and provides short-term relief and assistance in patient care delivery in all areas. The Coordinator obtains emergency supplies, equipment and medication, provides administrative and managerial functions in the absence of administration or other Nurse Directors, and provides feedback to department managers. The Coordinator also initiates and coordinates investigation and resolution of complaints occurring within assigned shift as well as participates in staff training and education. In addition, the Patient Care Coordinator effectively interacts with patients, family members and other health team members while maintaining standards of professional nursing.

MAJOR JOB FUNCTION

  1. Provides leadership, assistance, experience and mentoring to all patient care departments and adheres to the policies and guidelines of Tomah Health during times of decreased staffing.
  2. Serves as Rapid Response Clinician with critical care expertise in house during the off shifts to assist in the evaluation and treatment at the bedside of rapidly deteriorating patients.
  3. Provides expert guidance in emergency detentions, restraints, limitations of treatment, abuse/grief counsel, sentinel events, advance directives, organ procurement and patient transfers in accordance with EMTALA.
  4. Performs staffing activities to obtain and/or redistribute staff members. Conducts periodic rounds to assess unit activity and patient care needs. Executes call-off and on-call policies fairly. Contacts appropriate personnel in event of power outage, alarm activation or equipment failure. Coordinates patient care activities in the initial stages of a disaster. Assists with arranging transfers, as needed.
  5. Effectively makes independent decisions when there is no established policy or regulation to meet the situation or circumstances.
  6. Works independently as necessary, establishes priorities and reliably follows through to complete tasks. Accepts all assignments willingly and completes them in a timely manner. Interact with medical staff to build professional relationships among health care providers to support quality patient care and service. Initiates the chain of command process when appropriate.
  7. Promotes a positive environment in which the Patient Care Team can work cooperatively toward established objectives. Maintains communication and promotes positive interaction between all departments. Delegates responsibilities as appropriate. Serves as a line of communication between shift personnel and the CNO and the appropriate Nursing Director regarding specific problems or incidents.
  8. Submits written daily end report of shift for all hospital activities to the Chief Nursing Officer.
  9. Acts on performance/behavioral issues that pose immediate threat or harm to staff and/or patients.
  10. Participates in mandatory and other continuing education. Assists with Nursing staff orientation and annual training including but not limited to safety, security, hazardous materials and wastes, emergency preparedness, life safety, medical equipment and utility systems.
  11. Collaborates with Nurse Directors in the staff development of nursing and patient care strategies, goals and programs.
  12. Investigates all injuries, incidents and/or medical/health care errors in a timely manner. Completes incident reports, including those from actual accidents/incidents, near misses and/or medical/health care errors in a timely manner. Insures appropriate Administrative notifications are done in a timely manner.
  13. Serves as a member of appropriate committees as assigned. Monitors documentation of care given by participating in record review audits.
  14. Attends staff meetings and inservices. If unable to do so, receives all unit minutes and reviews with specific manager.
  15. Assists with patient care in patient care areas on a short term basis as needed. Evaluates patient care in acute care departments; ensures compliance with professional standards of nursing practice.
  16. Demonstrates the use of the nursing process on a wide range of patient ages, from pediatrics and adolescents through adults and geriatrics.
  17. Provides breaks for departments as needed. Provides oversight of the patient care environment.
  18. Demonstrates proficiency in regards to the computer documentation of patient cares and any other related computerized patient information/documentation.
  19. Performs other related duties as assigned or requested.

EDUCATIONAL REQUIREMENTS

  1. Graduation from an accredited school of nursing, BSN preferred.
  2. Current licensure to permit to practice as a registered nurse in the state of Wisconsin.
  3. Current certification or eligibility to become BLS, ACLS, PALS, and NRP certified.
  4. Fetal Monitoring Class – basic.
  5. 3-5 years nursing experience preferred.
  6. Prior supervisory experience preferred.

QUALIFICATIONS/SKILLS

  1. Excellent verbal and written communication skills.
  2. Excellent human relation skills as demonstrated by the ability to interface positively with all customers and employees.
  3. Must acquire the knowledge and maintain established competency levels to perform essential professional nursing practice.
  4. Must have proficient computer skills with the ability to operate within a Windows environment.
  5. Must have the ability to work with frequent interruptions, under stress, with minimal supervision, and to exercise initiative and judgment in analyzing, organizing, planning, prioritizing, scheduling and coordinating work with others.
  6. Needs to possess as high level of intelligence, integrity, sense of responsibility and ability to comprehend and analyze details.
  7. Knowledge of theory, principles, techniques and practice of professional nursing; of scientific principles and nursing research; of preparation, administration, and actions of medications; of treatments and medical procedures; of counseling and teaching techniques; of medical equipment as it relates to patient care.
  8. Ability to operate and handle many kinds of complex equipment.
  9. Ability to demonstrate clinical competence and maintain current knowledge in caring for infant through geriatric patients.

JOB REQUIREMENTS

  1. Regularly required to use hands to finger, handle or feel objects, and talk and hear.
  2. The House RN Coordinator is frequently required to stand, walk and reach with hands and arms. The House RN Coordinator is occasionally required to sit, climb or balance, stoop, kneel or crouch.
  3. The House RN Coordinator must occasionally lift and/or move up to 25 pounds.
  4. Good hearing is necessary to receive detailed information through oral communication and to make fine discriminations in sound.
  5. Visual acuity is needed to assess color changes, to verify accuracy of written materials and to administer and accurately prepare medications.
