SUMMARY
Serves as a patient advocate, guiding patients through the process in a timely and cost effective manner while maintaining a high standard of care. Monitors admissions establishing medical necessity, ensuring the appropriate level of care and facilitating the transition to an alternate post-acute care setting when appropriate. Conducts QA/QI screening to support the medical staff QA/QI program. In conjunction with the patient, support network, and members of the multidisciplinary team, formulates an appropriate discharge plan.

ATTRIBUTES
Minimum Qualifications: Education/Training: ASN with minimum of 3-5 years of Care Management experience. Bachelor of Science in Nursing or related healthcare field preferred. At least three years clinical experience in direct patient care.
Licensure/Certification: Current Registered Nurse licensure in the State of New York required. Annual BLS certification required.
PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
LICENSES/CERTIFICATIONS:
RN - Registered Nurse - New York State Education Department (NYSED)
BLS - Basic Life Support - American Heart Association (AHA)
PAY RANGE: $38.25 - $52.50
