Facility: WAB - Wellstar Administrative Building
This position requires travel between Wellstar Physical Therapy locations!
The Speech-Language Pathologist evaluates patients and develops individualized care plans addressing speech, language, voice, cognition, and swallowing needs in collaboration with the physician. Services are delivered across the full continuum of care in a cost-effective, quality-focused, and customer-oriented manner. Compliance with all regulatory standards is required.
Core Responsibilities and Essential Functions:
Patient Assessment:
Collaborate with physicians to establish and continuously evaluate treatment programs, care plans, goals, and discharge plans related to speech, language, cognition, voice, and swallowing.
Develop care plans in conjunction with patients and/or their families.
Create realistic discharge plans incorporating patient and/or family goals.
Treatment Planning and Provision of Care:
Implement speech-language therapy treatment plans.
Select appropriate treatment activities to help patients achieve their goals.
Revise goals and care plans with input from patients and families.
Provide ongoing education and training to patients and families.
Offer discharge instructions, follow-up, and referrals to community resources as needed.
Act as a resource for staff, families, and physicians on rehabilitation services and equipment.
Documentation:
Document evaluations and care plans according to departmental guidelines.
Record each treatment session following departmental protocols.
Complete and submit required documentation, including family education and discharge information, in accordance with facility and program guidelines.

Required Minimum Education:
Graduate from an accredited speech-language pathology program.
Master's Degree in Speech-Language Pathology or a related field.
Required Minimum License(s) and Certification(s):
Speech-Language Pathologist License.
Basic Life Support (BLS) or BLS Instructor certification.
Required Minimum Skills:
