40 hours per week - Monday through Friday 9:00AM to 5:00PM
Key Responsibilities:
Train and support Residential Group Home Managers on healthcare related processes
Monitor medical appointments for individuals in Residential Group Homes through their healthcare provider’s portal and BCL’s internal health record system
Assist Residential Managers with preparing for medication audits
Attend staff meetings and support Manager/staff with MAP and medically related issues
Assist individuals and staff during medical appointments as needed
Train and monitor Residential Managers performance related to communication with healthcare providers and documentation before and after medical appointments
Effectively communicate with healthcare providers as needed
Assist in training of new Managers/staff with regard to medical appointments, MAP policies
Assist agency nurses with tasks such as running reports in ICentrix, monitoring preventive health screenings and gathering information from healthcare providers
May be required to provide management coverage at group homes in the absence of assigned manager
Working Conditions:
Physical activities include but not limited to climbing, balancing, stooping, kneeling, pulling, etc.
Exerting in excess of 100 lbs. of force occasionally.
Subject to both indoor and outdoor environmental conditions.
The worker is subject to hazards such as moving parts, moving vehicles, etc.
Must be able to push up to 250 pounds using wheelchair, carry up to 25 pounds
Required Qualifications:
High School/GED
All applicants must be at least 18 years of age and have had a current US drivers license
MAP Certification for at least 1 year
At least 2 years experience working with individuals with Intellectual and Developmental Disabilities in a Residential or Day Program setting
Must complete all BCL trainings and other training as assigned
