Licensed Practical/Vocational Nurse (LPN/LVN)


Full-Time | Monday - Friday, 9:00 am - 5:00 pm











Job Summary:

We are looking for a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care to patients in their homes. In this role, you will work under the supervision of a Registered Nurse, following physician orders and care plans to ensure quality patient outcomes. This position is an opportunity to provide compassionate care while working in a supportive and collaborative environment.










Key Responsibilities:

• Administer nursing care according to patient care plans.


• Monitor patient reactions to treatments, and medications, and report any changes to the RN and physician.


• Teach patients and families self-care and health practices to support their recovery and well-being.


• Maintain accurate records of patient care, treatments, and observations.


• Participate in patient care meetings and collaborate with other healthcare team members as necessary.


• Ensure adherence to safety protocols and aseptic techniques.

Qualifications:

• 
  Education: Graduate of a state-approved practical or vocational nursing program.


• 
  License: Valid LPN/LVN license in the state of practice.


• 
  Experience: At least 1 year of nursing experience preferred.


• 
  Skills: Ability to work independently, exercise good judgment, and align with the agency's philosophy.

Work Hours and Schedule:

• 
  Hours: Monday - Friday, 9:00 am - 5:00 pm


• 
  On-Call: Nights, weekends, and holidays as needed.
