You will be involved in a new Program of All-Inclusive Care for the Elderly model-centered on the belief that the well-being of seniors with chronic care needs and their families are better served in the community.
Bring your expertise and skills as a member of the IDT to help manage the complex medical, functional, and social needs of older adults as we implement our Northern Kentucky PACE program. Ultimately, PACE will work to maximize the dignity and unique needs of seniors and their caregivers
Wage- $17.00-$21.00 per hour based on experience.

Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
Must have a valid driver's license, proof of insurance and have means of transportation.
Must pass physical examination
Must pass background check and drug screening.
Education: High school diploma or equivalent required; Valid Driver's License in state of hire.
Have a minimum of one year experience in working with the frail and elderly population.
