Job Summary:

The Hospital Care Manager coordinates patient care for Hospital Services and Therapeutic Foster Care from initial assessment through discharge, acting as a liaison between the patient, patient’s family, and other professionals and agencies. They are responsible for preparing treatment plans and discharge plans, coordinating actions of the multi-disciplinary treatment team members. They coordinate treatment conference, transfer to and from the facility, and outpatient appointments after discharge.

Qualifications:

• Ability to communicate effectively and professionally with patient, patient’s guardian, treatment team, and all referral sources.
• Knowledge of community resources.

EDUCATION AND EXPERIENCE:
• BSW, although LSW is preferred.
