In collaboration with the providers completes Medicare Annual Wellness Visits with a goal of 6per week
In collaboration with the practice manager assists the staff with pre-visit planning to complete tasks associated with closing gaps in care and meeting quality measures.
Serves as a clinical resource/educator to facilitate the development of the team in collaboration with the provider, practice manager, clinical leads and care manager.
Reviews quality measures with the Director of the Medical Group, Program Consultant, and Director of Community and Population Health.

Qualifications
Bachelor's preferred (or in progress).
3 years of clinical nursing experience
1 year of care coordination and/or understanding of primary care practice dynamics.
Current Maryland Nursing License
Care Management Certification, preferred
BLS, required
