Job Summary:
The Clinical Pharmacy Specialist - Emergency Medicine is responsible for providing comprehensive pharmaceutical care to Emergency Department (ED) patients through clinical services, formulary management, development of policies and procedures, and integrated drug distribution. The primary job responsibilities of this position include providing pharmaceutical care to ED patients, providing drug distribution support, educating ED personnel and serving as the pharmacy representative on appropriate Emergency Department and hospital committees.
Core Responsibilities and Essential Functions:
Drug Knowledge and Bedside Clinical Practice a.Evaluate and monitor drug regimens for appropriateness. Interpret physicians orders and screen them for various types of interactions. b.Participate in medical emergencies (codes, STEMI, med alerts, traumas, strokes) within the emergency department. c.Provide timely and accurate toxicology and drug information to patients, nursing, and medical staff. d.Assist nurses and physicians with medication reconcilliation for patients being admitted to the hospital from the emergency department. e.Assist physicians with appropriate discharge prescriptions. f.Provide recommendations and/or manage therapy for antibiotics or other medication regimens as prescribed by the provider. g.Prepare IV admixtures, compound, and repackage a wide variety of pharmaceuticals. h.Enter/verify orders in the computer system and appropriately dispense medications Clinical/General Pharmacy Practice a.Serve as pharmacy liaison to the emergency department for various quality and safety initiatives. b.Serve as pharmacy liaison to the emergency department to help maintain regulatory readiness and assist with process improvements. c.Monitor patients and processes for potential Adverse Drug Reactions and Medication Errors. d.Participate as a member of the Wellstar Pharmacy and Therapeutics (PT) Committee, as well as other System and facility-specific committees as assigned or as deemed necessary. e.Identify target medications/therapies, develop criteria, and implement data collection for Medication Use Evaluations (MUE). f.Monitor quality metrics related to emergency medicine patients and develop improvement strategies as necessary. g.Assist in formulary maintenance by critiquing new entities, comparing medication classes, and developing therapeutic interchanges where applicable. h.Document clinical interventions on an ongoing basis. i.Assist in developing drug protocols. Educational Responsibilities a.Provide patient, nursing , and medical staff education when needed or requested (include medical residents, where applicable). b.Serve as a preceptor for the Pharmacy Residency Program (where applicable) as well as students from affiliated schools of pharmacy.
Required Minimum Education:
Bachelor's Degree from an accredited college of pharmacy Required or
Doctorate Pharm D from an accredited college of pharmacy Preferred
Required Minimum License(s) and Certification(s):
All certifications are required upon hire unless otherwise stated.
