We are looking for someone to be part of our team as a Social Worker II. In this role, you will be an integral part of our team working with adult patients with mental illness, including those that have sexually offended at the Norfolk Regional Center.

The Department of Health and Human Services at the Norfolk Regional Center is seeking a compassionate and experienced Social Worker II to join our team. Under general supervision, this position will focus on providing basic social work services and discharge planning. You will assist clients with personal issues involving financial, health, family, and environmental adjustment, as well as serve on interdisciplinary teams to determine and implement appropriate programs to meet client needs. May provide psychoeducation sills training groups. Work will be performed under the supervision of a Certified Master Social Worker.
In addition to standard social work duties, the Social Worker II will be responsible for providing social work intervention input for treatment planning, completing weekly and monthly progress notes, and performing discharge planning and referrals. This position will serve as the Treatment Plan Coordinator for assigned patients and coordinate with the Social Work Office Specialist to register visitors, obtain releases of information, and explain visitor processes and policies. The role also includes auxiliary social work duties such as obtaining necessary documents, providing education on program guidelines, and completing paperwork for items purchased by the Norfolk Regional Center for clients.
Key Responsibilities:
Provide Social Work input for treatment planning.
Complete weekly and monthly social work progress notes
Collect and analyze social history data to assess educational, vocational, and social needs of clients, and determine eligibility for services.
Serve as an advocate for clients, coordinating referrals to appropriate service agencies.
Negotiate provider contracts and evaluate services such as child care, transportation, and homemaker aides.
Participate in interdisciplinary team meetings to plan and implement client care programs.
Involve clients' family/guardians in treatment planning, program implementation, and referrals.
Conduct discharge interviews and develop client discharge plans by completing referrals to aftercare agencies and ensuring timely financial and placement arrangements.
Lead individual and group counseling sessions and social skills programs to support client transitions to nursing homes and residential settings.
Maintain detailed documentation of client social work activities and keep accurate records.
Hours:
Monday-Friday 8:00AM-5:00 PM.
Requirements / Qualifications:
Minimum Qualifications:Bachelor's degree in Social Work with certification in Social Work and one year experience in the field of employment, OR a Bachelor of Arts degree in a related field with certification in social work by grandfathering and two years of experience in a residential setting, public welfare, or community service work.
Other: Certification as a Social Worker by the Nebraska Department of Health is required.
Hiring Rate:
$23.335 Per hour + an additional $3 facility differential.
Knowledge of:
Case management and group therapy theories and techniques.

Advocacy theory and therapeutic intervention techniques.
