This job will have the following responsibilities:
Reliable transportation to get to patients and the office for mandatory meetings.
Professional Management of the Patient and Family
Communication with Medical Professionals, Home Health, and staff.
Maintaining Accurate, Up-to-Date Records.
Training the Patient and Caregiver.
Elevating Necessary Matters to the Attention of the Patient Care Manager.
Able to properly diagnose the patient's condition.
Has efficient and effective writing skills to write thorough notes on patients.
Regularly re-evaluates patient condition.
Evaluates and measures performance of home aides.
Works collaboratively with the interdisciplinary team to ensure positive patient outcome.
Submission of Oasis within 24-48 hours after the first Start of Care
Obtains medical history, particularly as it relates to the present condition, from the patient and/or family member.
Conducts a brief physical examination of the patient, including vital signs, ambulatory status, mental status, bowel and urinary regularity, appetite, and type of diet.
Interprets nursing services of the agency to patients and families as part of planning of care.
Provides skilled care and maintains confidentiality of client's situation, records and services provided according to HIPAA regulations.
Evaluates the effectiveness of his/her nursing service to the individual and family.
Submits all patient notes and/or visit records on a timely basis.
Develops, implements, participates in the nursing care plans, and coordinates total patient care.
Qualifications & Requirements:
Current Registered Nurse license in the state of Arizona.
Two (2) years' experience in a home health and/or related setting preferred.
Must be CPR certified.
Valid State Driver's license
Current Fingerprint Clearance Card
Case management experience
Reliable vehicle to travel.
