El Futuro is currently hiring a full-time, bilingual Case Management (Conexiones) Manager to lead the clinic’s internal case management program, Conexiones, in our Durham and Siler City locations. The Conexiones Manager provides direct services to clients and leads the program by supervising Case Managers, assigning caseloads, coordinating internal data tracking and case management impact assessment, conducting comprehensive needs assessments, creating and implementing case management plans, and managing communications related to Conexiones within El Futuro and with external partners.
This position works in-person, with the Durham Clinic as the primary location and in Siler City as needed, with flexibility to work remotely one or two days per week. The typical work schedule is Monday through Friday 9:00am to 5:00pm with occasional hours in the evenings and /or weekends, and must be available for Friday morning meetings, 9:00-11:00am (mostly virtual).

ESSENTIAL FUNCTIONS
The Conexiones Manager performs a wide range of functions including, but not limited to the following:
Conexiones Manager:
Assign caseloads to Case Managers, typical weekly caseload is 20-30 cases.
Maintain comprehensive and timely documentation of case management services, including data for grants and contracts.
Supervise Case Managers through weekly meetings, maintaining detailed supervision logs, and conducting quarterly reviews of care coordination documentation.
Supervise Case Management Interns and develop intern job functions in accordance with academic institution requirements.
Coordinate comprehensive onboarding and ongoing training for Case Managers and Interns; provide Conexiones orientation for all new El Futuro staff.
Conduct bi-weekly Conexiones team meetings by setting meeting agenda and maintaining log of topics discussed.
Coordinate internal resource information sharing and stay informed about local community initiatives and allied service providers in Chatham, Durham, and Orange Counties.
Collaborate with El Futuro leadership to determine strategic direction of the Conexiones team.
Coordinate Conexiones virtual coverage in the Siler City clinic.
Case Management:
Provide family-centered, evidenced-based, culturally-informed case management services for a caseload of approximately20 clients.
Communicate and collaborate with the El Futuro service provider team (i.e. therapist, psychiatrist) to ensure clients receive coordinated delivery of services.
Maintain comprehensive and timely documentation of case management services, including completing data for Governor’s Crime Commission (GCC) reporting.
Facilitate care coordination between El Futuro and allied community organizations (i.e. schools, DV/SA services, legal services, etc.) serving Latine victims of crime.
Advocate for improved access to community resources and opportunities for El Futuro clients.
Develop individualized Service Plan with clients, using Contextual and Motivational Interviewing methods.
Coordinate post-case management survey completion and participate in community partnership meetings.
Stay informed of changes in delivery of services and new initiatives by allied service providers and communicate to team and staff as needed.
Participate in periodic skills-based training to maintain best practice approaches for working with Latinx victims of crime.
Create plans of action with clients in caseload (strengths, urgent needs, and short-term case management goals).
Maintain client confidentiality in compliance with HIPAA regulations.
General Responsibilities for El Futuro Staff:
Participate in meetings and organizational related activities as directed by the supervisor, which may include All Staff Meeting, Individual Supervision, participation on a committee, or other activities.
Play an active role in the continued development of El Futuro’s work via participation in fundraising and other agency-wide activities.
Engage in learning opportunities through “La Mesita”, El Futuro’s training platform and network of bilingual providers in North Carolina, or through other appropriate career development opportunities.
EDUCATION and/or EXPERIENCE
Bachelor’s Degree in Social Work, Counseling, Psychology, or related field preferred.
Minimum of 2 years of experience in case management, care coordination or similar work, preferably in a mental health or healthcare setting working with the Latine community.
Required: Fluency in spoken Spanish. Must also be able to read and communicate with clients in written Spanish. Must be able to document clinical encounters in English.
KNOWLEDGE, SKILLS, ABILITIES
Familiarity with local resources and services available to the Latine population in El Futuro’s service areas or initiative and willingness to learn.
Strong leadership and supervisory skills, with the ability to motivate and support a team of Case Managers.
Demonstrated ability to work effectively with diverse populations, including individuals with mental health and substance use disorders.
Ability to manage multiple tasks and priorities while affording attention to detail and organization.
Flexibility, creativity, and adaptability to meet the changing needs of clients and El Futuro.
Proficiency in Microsoft Office, electronic health record systems, and project management systems (El Futuro uses IMS and monday.com).
