Perform Non-medical case management of Soldiers with permanent medical conditions for entry into the Disability Evaluation System (DES) requiring a Medical Evaluation Board (MEB), Non-Duty Physical Evaluation Board (NDPEB), or Military Occupational Specialty (MOS) Administration Retention Review (MAR2)
Perform Non-Medical Case Management of Soldiers with temporary medical conditions to either Return to Duty (RTD), RTD with limitations, or to Medical Retention Determination Point (MRDP) and entry into the DES Services
This is designed to be flexible and responsive to meet ever-evolving medical readiness requirement of the Texas Army National Guard (TXARNG) in support of both State and Federal missions. The TXARNG requires a highly responsive process whereby the government can quickly and consistently obtain required support and services.
QUALIFICATIONS
LVN shall possess qualifying certificate or degree from an educational institution accredited by the Texas Board of Nursing
LVN shall possess an active, unrestricted, and unencumbered license issued by the Texas State Board of Nursing
Case Management certification is preferred but not required
DUTIES
Follow the regulations and policies applicable to Individual Medical Readiness (IMR) and deployment
Provide weekly Profile Reports and Brigade Case Load Reports to Director of Case Management by end of week
Resolve and manage non-medical ready status by returning Soldiers to medically ready status or forwarding records for a Fit for Duty (FFD) Determination for Soldiers in a non-medically ready status within 7 business days
Maintain Health Insurance Portability and Accountability Act of 1996 (HIPAA) standards and guidelines for all health information obtained and used during this contract
Perform initial interviews regarding deployment limiting conditions for Soldiers processing through FMRs and SRPs within 30 days of case management notification
Educate Soldiers on available resources to improve medical and dental readiness
Ensure all medical documentation and release of information received from the Soldier and/or their medical team is received and placed in appropriate electronic record of Health Readiness Records (HRR) within 5 days of initial interview
Review and upload treatment plans for Soldiers in a non-medically ready status into HRR and MEB Prep within 5 days
Submit medical documentation and profile concerns to military provider for updates within 5 business days
Review assigned cases and identify Soldier’s needs with appropriate medical facilities and ensure medical and administrative charting is completed in eCase monthly
Track medical appointments by communication and coordinate with unit to ensure Soldier attendance and submission of follow up documentation for all assigned cases
Log any contact with Soldier via phone, face-to-face, or email in eCase within 1 day of encounter
Prepare all required medical and administrative documents required for medical evaluation for Soldiers identified for board proceedings
Monitor cases in Medchart monthly throughout the evaluation process until completion of Soldier being RTD or administratively separated from the military
Distribute final board proceeding to MRNCO by email within 5 days of receipt
Develop a QCP to properly evaluate roles, responsibilities, and procedures for desire results
Monitor iPerms monthly for discharge orders and closes case within 5 days as orders are received
Provide all administrative and/or medical charting in eCase. Charting shall be completed within 5 days of receipt of information
Record Management:
Confirm all medical documentation received from the Soldier and/or medical team, as well as the Soldier’s hard copy record, is placed in HRR upon receipt
Maintain Soldier medical records in accordance with Army Regulation
Adherence to Regulations and Policies:
Follow the regulations and policies applicable to Individual Medical Readiness (IMR) and deployment
Medical:
Assist Soldiers in the Medical Screening program in conjunction with the Deputy State Surgeon (DSS) and Director of Case Management, track and assist in the guidance of the Soldier’s care from initiation of non-retainable condition in accordance with Army Regulation to Soldier RTD or referral to the DES. Assist Director of Case Management and DSS in Fitness for Duty (FFD) Determinations, MEB, Non-Duty Related Physical Evaluation Board (ND-PEB), MAR2 and Medical Administrative Separation. Assist the TXARNG Medical Readiness Detachment (MRD) in maintaining Soldier’s medical records in accordance with Army Regulation. Use of various software programs to maintain database files and prepare reports. Provide liaison services and coordinate with the Director of Case Management DSS on the most difficult cases. Facilitate case management initiatives and deployment operations NLT 10 days of receipt and sooner as needed.
Possess working knowledge of Microsoft Office applications and the ability to monitor cases, keep records, consult with military providers, educate patients, record vital signs, and reporting symptoms and changes in deployability. Maintain accurate, complete health care records and reports. Record patient vital signs and medical information in Periodical Health Assessment. Assess, implement, or evaluate profiles by working with the healthcare team members. Review medical records for diagnosis requiring Medical Boards Evaluations. Conduct monthly and as needed case review with representative(s) of the TXARNG Commands, Health Services, and other TXARNG agencies with support from DSS.
Case Management:
Possess working knowledge of Microsoft Office application and the ability to understand and translate medical terminology and applications into various databases as appropriate quickly and accurately with less than 1% data entry errors. Maintain Soldiers’ medical/dental records in accordance with all regulations.

Duty Hours: 0700-1700 Monday through Friday (telework is not authorized)
