Summary:
Coordinates the discharge of patients.
Responsibilities:
Performs clerical and customer service functions in support of discharge efforts.Participates in the completion of required regulatory documentation for governmental agenciesParticipates in discharge planning including coordinating patient transfers to other departments or facilities, and coordinating community resources. Provides discharge education and resource referrals to patients.Secures post acute care placement of patients.Communicates with insurance companies/charities to ensure that cost of required care arrangements are covered.
