The POSITION
Under general supervision of the Deputy Director of Housing, within Alameda County Health's (AC Health) Housing and Homelessness Services (H&H), this position is responsible for direction and management of a complex health care program (CalAIM Housing Community Supports), oversight of program staff, including strategic planning and sustainability planning to expand housing navigation and tenancy sustaining services for individuals experiencing homelessness in Alameda County. Primary responsibilities of the Housing Community Supports Program Manager (Health Care Program Administrator II) include planning, direction, staff oversight and management to ensure quality implementation of new programs and services, leveraging multiple funding streams. This role requires strong analytical evaluation skills to facilitate analysis of program effectiveness following both an equity-centered, and data-informed approach.
DISTINGUISHING FEATURES
This position is in Alameda County Health H&H and reports to the Deputy Director of Housing. Incumbents in this position are responsible for managing a large complex health care program with multidisciplinary staff and multiple funding sources. The Housing Community Supports Program Manager collaborates with other County agencies/departments, Managed Care Providers and funders, people currently or recently experiencing homelessness, and community organizations on programmatic development, strategic planning, and management of funding related to its assigned area. This position oversees and directs the activities associated with supportive services for residents experiencing homelessness in Alameda County.
EXAMPLE OF DUTIES
Oversees and directs, through subordinate supervisory and professional staff, administration of contracts for the direct delivery of Housing Community Supports (HCS) Program activities associated with supportive services for residents who are homeless or at risk of homelessness.
Directs the review and evaluation of the HCS program, funding sources, service delivery and other elements for possible program inclusion; directs needs assessment and pilot projects and submits documentation and recommendations.
Plans, supervises or directs, reviews and evaluates the work of supervisory, professional, technical and support staff; selects staff and provides for their training and professional development; implements strengths-based approaches.
Oversees the coordination with other organizations, including County departments, community-based organizations, housing organizations and cities to identify potential program sites for services funded within H&H.
Provides for the development and delivery of appropriate training for clients, family members, care providers and others in how to find, access, and retain appropriate housing and services.
Monitors legislative activities and changes in laws and regulations; assesses their effect upon program activities; develops and implements policies and procedures to ensure compliance with such changes.
Oversees the collection and publication of financial, services and outcome data regarding the performance of H&H related investments, services and functions.
Represents H&H at national, state, county and community meetings related to housing and supportive services.
Performs other duties as assigned.
HCS Program Manager (Healthcare Program Administrator II)
Possession of a bachelor's degree from an accredited college or university in business administration, health care administration, public health, social welfare or related field.
(Additional experience as outlined below may be substituted for the education on a year for year basis.)
AND
Experience:
The equivalent of four years full-time professional experience in program administration, management, planning and evaluation in the oversight of direct delivery of health care services to clients equivalent to the Alameda County's class of Health Care Program Administrator I.
Substitution:
A master's degree in business administration, health care administration, public health, social welfare or related field may be substituted for one year of experience.
License:
Specified positions may require possession of a valid California driver's license. Specified positions may also require licensure or certification in the programmatic area to which assigned.
