VCU Health System is seeking a full time Director to manage clinical operations within its Continuum Integration Center's network of connected clinical resources engaged across the Richmond region in Post-Acute Care (PAC).
The position develops, implements and monitors service-line specific operating standards at all levels across the VCUMC post-acute Continuing Care Network (CCN), within owned, affiliated, or contracted (30+) entities. These include joint venture relationship management with VCU Home Health, VCU Hospice, and VCU Sheltering Arms Institute.
The position also supports enhanced care coordination and flow between VCU Medical Center and non-acute providers to improve overall quality of care and reduce total health spending. These involve VCU's PAC Funding Support programs (including indigent care), Centralized PAC Placement team, PAC Continuing Care Transitional Navigation programs serving CMS Bundles patients, Project RED readmission reduction, and a 24/7 Ask-A-Nurse Line.
The Senior Services | Continuum Integration | VCU Health at Home service line model operates with a patient-centric focus and a comprehensive, team-based, outcomes-motivated approach to care, using population health principles. The position and service line support the "Quadruple Aim" of improving the patient experience (including quality and satisfaction), improving the health of populations, reducing the per capita cost of healthcare, and improving provider satisfaction.
The lead operational manager for PAC services, this experienced health leader will be responsible with Senior Director for planning, strategizing, organizing, leading, and controlling a broad and diverse group of services and functions in transitional, home and community-based care. The position requires education and experience enabling the successful applicant to provide management, assessment, planning and innovation. They also need to be able to work seamlessly with health providers, supervise staff members, and modify programs and policy to meet emerging needs for individual patients as well as the organization and community as a whole.
This position requires the Director to serve as a resource and leader in the service line for patients, staff, health care providers, management, legal, and other disciplines. This leader will also help develop long and short-range plans in alignment with the overall VCUHS strategic plan and will provide support to Executive and Medical Director leadership in the design and implementation of new community-related initiatives.
Duties:
The Director of Continuum Integration works collaboratively with senior leadership across the VCUHS Enterprise to coordinate and provide high level management and leadership for Continuum Integration/Post-Acute care, quality, and management, across VCU Health markets. This includes all components of Post-Acute Care (PAC) resources as defined by MedPac (CMS) as being rehabilitation or palliative services that beneficiaries receive after, or in some cases instead of, a stay in an acute care hospital. Depending on the intensity of care the patient requires, treatment may include a stay in a facility, ongoing outpatient therapy, or care provided at home.
The Director of Continuum Integration organizes and leads efforts of the VCUHS Continuum Integration Center (CIC). This position supports enhanced coordination between VCUHS general acute-care hospitals and non-acute providers to improve overall quality of care and reduce total health spending.

The position and service line also supports the "Triple Aim" of improving the patient experience (including quality and satisfaction), improving the health of populations, and reducing the per capita cost of healthcare.
Supervisory responsibilities:
Direct oversight of VCUHS Continuum Integration Center care coordination staff and clinical operations including 24/7 care coordination hub, telehealth clinic, and post-acute care delivery across Continuing Care Network of owned, affiliated, or contracted providers
Experience REQUIRED:
Minimum of 7 years health service experience with significant HR, financial and operations, responsibility.
Minimum 3 years of experience in a supervisory role.
Demonstrated leadership and, initiative in healthcare service excellence and business development
Strong quantitative analytical skills
Experience developing and implementing initiatives and processes and manages to goals for clinical services that align with organizational and operational strategies in post-acute and community care.
Bachelor's Degree in Health Administration, Business, Nursing/Health sciences, Public Health, Management, or related field.
Experience PREFERRED:
10 years health service experience with significant HR, financial, and operations responsibility.
Prior experience in Home Health, Skilled Nursing Facility (SNF), Long Term Acute Care Hospital (LTACH), Inpatient Rehab Facility (IRF), or Hospice care delivery
Master's Degree in Health Administration, Business, Nursing/Health sciences, Public Health, Management, or related field.

Extended hours and weekend coverage may be required
