The Primary Therapist is responsible for performing client care activities as directed by the Clinical Director. He/she/they will provide group and individual therapy as ordered by the Clinical Director. They actively collaborate with the Treatment Team in the formulation of the Master Treatment Plan, ongoing treatment, and assists in coordinating discharge planning.
Actively participates in Multidisciplinary Treatment Plan meetings.Shares pertinent information with team as obtained from clients family, or others as indicated throughout program.Attends daily and weekly staffing meetings per schedule.Evaluates the impact of therapeutic interventions upon the therapeutic milieu.Facilitates group therapy and completes subsequent documentation in a timely manner.Performs individual therapy with client/family. Caseload will include co-occurring clients.Assesses clients using ASAM and diagnose using current DSM to determine need and appropriate level of care, continued need for services and transfer/discharge. Obtains any previous necessary records to accurately assess needs.Collaborates with client to develop a treatment plan and revises as needed.Creates transfer/discharge plan as required.Performs in a professional manner and role-models positive mental health and/or recovery maintenance.Obtains Releases of Information for care coordination purposesCommunicates and coordinates with any necessary outside sources, such as mental health treatment providers, criminal justice agencies, schools, employers, child protective services, etc.Documents the assessment of client progress/regress and interventions provided in the medical record, on an ongoing basis.Recognizes, responds to, and assists in psychiatric crisis situations and provides the intervention necessary.Responds and assists in medical emergency situations as directed and needed.Maintains personal education as it relates to evidence-based and evidence-informed clinical practice and patient careReports personal symptoms of suspected illness or contagious diseases to the Infection Control Designee.Performs all other tasks as assigned. Demonstrates the ability to communicate and effectively interact with people across cultures, ranges of ability, genders, ethnicities, races, etc.; demonstrates the ability to successfully deliver culturally responsive services; and manages any cultural related issues of clients as it pertains to the intern position.Maintains understanding of patients rights per the Mental Health Code, upholds patients rights, reports abuse of patients rights immediately and assists patients with filing complaints, as needed.
