Scope of Position/Position Summary:

The Case Management & Social Work Manager is responsible for overseeing daily operations and promoting teams in case management and social services from pre-admission to post discharge by directing Clinical Case Managers, Utilization Review Case Managers, Social Workers, and Office Associates within aligned teams across OSU, Brain & Spine, Ross, Dodd, OSU East hospitals and emergency The manager is responsible for assuring teams facilitate and coordinate high quality, safe, timely, efficient and effective social services and discharge planning and as applicable. The Manager works closely with the Director to assure innovative strategies are used positively to impact patient care, throughput, achieve quality patient outcomes; improve service performance and reimbursement, and length of stay and costs.

Minimum Qualifications
:

For Hire - Bachelor's degree required, Masters preferred. Registered Nurse or Licensed Masters Social Work required. 5 years clinical experience applicable to the patient population in acute care setting, home care, hospice, community agency, and/or payer relations required. Prior demonstration of leadership or management skills strongly preferred. This role must understand the health care continuum and have the ability to solve complex problems and possess strong communication and interpersonal skills. Must be able to work with various computer applications. Case Management Certification preferred.
