Overview
The FOU Care Facilitation RN provides various direct care services including triage services, patient advocacy, needs assessments, symptom management, care facilitation, education, nursing advice, coordination of necessary medical services within the OHSU system, as well as system navigation and networking, resource referrals and connections within the surrounding health communtity, organizations, and agencies. The FOU Care Facilitation RN works with patients within the inpatient setting, as well as the ambulatory settings. The FOU Care Facilitation RN requires strong and current nursing skills to prioritize care needs, to guide and facilitate patient care, and to provide linkages to appropriate OHSU services.
The FOU Care Facilitation RN provides ongoing communication with the FOU patients and families, and supportive collaboration with all members of the OHSU multi-disciplinary care teams. Assurance of service excellence for the FOU patient’s care experience is of utmost concern, emphasizing patient-centeredness.
The FOU Care Facilitation RN additionally participates in all OHSU Health TownHall meetings, OHSUF TownHall meetings, Dean’s Office Staff Meetings, and FOU Staff Meetings.
Responsibilities
BSN Graduates: Baccalaureate Degree in Nursing from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA) 30 days before start date.
ADN Graduates: Associate Degree in Nursing from an accredited program 30 days before the start date.
Associate degree Nurses required to enroll in BSN program within 3 years of hire and complete within 5 years of hire
Current, unencumbered Oregon State Registered Nurse License
BLS at time of hire and must be issued by the American Heart Association (AHA)
BLS must not expire during orientation
Must be able to perform the essential functions of the position with or without accommodation
Minimum of 4 years acute care required.
Demonstrated comfort dealing with high profile individuals required.
