Description: Judge Healthcare is currently seeking a Medical Communication Liaison for a HYBRID role in Nashville, TN for our biggest client!
Location: Nashville, TN (Hybrid: 1-2 days in the office per week)
Type: Contract W2- 10 months!
Work Schedule: Monday to Friday, 8 AM - 5 PM EST
Job Description:
The Engagement Specialist is responsible for engaging with members to remove barriers to Social Determinants of Health (SDOH) and serving as the communication liaison between our company and referral sources. Key responsibilities include:
- Engaging with members identified as having significant SDOH barriers.
- Collaborating with members to set goals and determine next steps for addressing SDOH.
- Assisting members in selecting appropriate providers or programs to support their needs.
- Coordinating with HCM to ensure individualized plans align with existing or needed case management efforts.
- Acting as a liaison between referral sources and the health plan.
Qualifications:
- BA/BS degree in Public Health, Social Work, Rehabilitation Counseling, or a related field.
- Minimum of 2 years of experience in marketing, job development, job placement, or social work, or an equivalent combination of education and experience.
- Knowledge of Vocational Rehabilitation preferred.
- Case management or customer service experience preferred
