This job will have the following responsibilities:

• Scheduling patients from hospital to home.


• Follow up appointments for the QTC patients.


• Medical Knowledge important


• Able to work with excel; entry


• Working with a team to schedule QTC appointments (Hospital to Home) for contracted and affiliated patients to aid in limiting readmissions.
